Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a)The G-2 methanol "kill system" delivery piping into the bottom of the Step II reactor vessels was not solely dedicated to the "kill system," nor was it car sealed open to ensure its availability in the event of a potential runaway reaction. Feasible and useful methods of correcting these hazards include, but are not limited to, the following: (a)Install dedicated kill systems and associated delivery piping on the G-2 Step II reactor vessels or implement an effective car seal program to ensure that the kill system is always available.
