29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the following sequence of actions were not followed in accordance with 29 CFR 1910.147(f)(1)(i) - (v): a/ Inside of the Fabrication Shop, employee(s) did not isolate the 480 volt electrical, hydraulic, or gravity energy sources belonging to Press Brake #1, unknown m/n or s/n, prior to changing out the die. Specifically, the employee(s) failed to apply the lockout devices, did not remove the lockout devices, then energize and proceed with testing or positioning of the die, and finally deenergize all systems and reapply all energy control measures to continue any additional servicing and maintenance.
