Text For Citation: 01 Item/Group: 001 Hazard:

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to struck-by and fall hazards while operating a scissors lift. On or about at 1/17/15 located at 1527 Augusta Ave., Augusta, GA: Employer failed to ensure that: 1. Steps has been taken to prevent a collision with the aerial platform within the area of travel of a Bridge Crane. 2. Operator training was provided to operate aerial platform (SJ1114626) including but not limited to loading, unloading, inspecting or any form of use. 3. Aerial platform (SJ1114626) operated on the firm level surface rather than unstable gravel surface. 4. The aerial platform (SJ1114642) given a visual inspection and functional test such as but not limited to tires and wheels, safety devices and missing pins from Guardrail System. No operating and maintenance manual was provided or stored on the aerial platform.
