29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative Location: 75 West End Avenue, New York, NY. Elevator Shaft. a) The employer did not establish a LOTO program to ensure that the elevators and associated equipment were not isolated from energy sources and rendered inoperative during the modernization of the elevators. On 1/9/2015, an employee was working inside the elevator shaft, at the lobby level, between shafts at the lobby level. Elevators 7,8 and 9 were not locked out while the employee worked in the shaft.
