29 CFR 1904.4(a): The employer did not keep records of fatalities, injuries and illnesses as required by this part: On or about December 18th, 2014, at the jobsite, 1 East 11th Street - Slip3, Riviera Beach Florida 33404, the employer did not keep OSHA 300 forms -Log of Work Related Injuries and Illnesses, 300-A form - Summary or Work-Related Injuries and Illnesses; and 301 form - Injury and Illness Incident Report specific for the site. a) On or about May 23, 2013, an employee was fatally injured at the worksite. b) On or about June 17, 2013, an employee suffered a leg and ankle injury. c) On or about March of 2015, an employee injured a leg when a gangway to a barge rolled over.
