29 CFR 1910.305(d)(2): Panel boards were not dead front: a) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to electrical hazards, in that inside a west-side wall-mounted 220 VAC panel-box No. 18 did not include a dead-front while employees activated switches, and performed other duties in the near-vicinity. b) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to electrical hazards, in that inside a west-side wall-mounted 220 VAC panel-box No. 19 did not include a dead-front while employees activated switches, and performed other duties in the near-vicinity. c) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to electrical hazards, in that inside a west-side wall-mounted 220 VAC panel-box No. 27 did not include a dead-front while employees activated switches, and performed other duties in the near-vicinity. d) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to electrical hazards, in that inside a west-side wall-mounted 220 VAC panel-box No. 3 did not include a dead-front while employees activated switches, and performed other duties in the near-vicinity. e) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to electrical hazards, in that a wall-mounted 220 VAC panel box (labeled 6 GR) on the east side of the computer room did not include a dead-front while employees activated switches, and performed other duties in the near-vicinity. Recycling Revolution, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.305(d)(2), which was contained in OSHA inspection number 589260, citation number 1, item number 12, and was affirmed as a final order on Feb. 13, 2013, with respect to a workplace located at 4190 Collins Road, Unadilla, GA 31091.
