29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to amputation hazards, in that the Ten-Sixty vertical baler's 7 ½ in. X 15 in. opening was not guarded as the operator inserted a pole into the loading chamber to loosen plastics when the ram cycled downward. b) At the recycling facility in Unadilla, GA: On or about November 12, 2014, and at times prior thereto, the employer exposed employees to amputation hazards, in that when the Cram-A-Lot and Ten-Sixty vertical balers access doors were open when rams cycled downward as operators reached inside the loading chambers to loosen Styrofoam and plastics pieces. Recycling Revolution, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 1910.212(a)(3)(ii), which was contained in OSHA inspection number 589260, citation number 1, item number 8b, and was affirmed as a final order on Feb. 13, 2013, with respect to a workplace located at 4190 Collins Road, Unadilla, GA 31091.
