29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: In the following locations, electrical equipment was found to have an open ground when tested: A. Main Room, Front of Shop: On, or about, November 25, 2014, a quad receptacle used to energize an extension cord and a reel type drop cord that in turn energized light fixtures, fans and portable tools. B. Main Room, Front of Shop: On, or about, November 25, 2014, a duplex outlet used to energize a Delta tool sharpener showed an open ground when tested. C. Side Room, Front of Shop: On, or about, December 1, 2014, a reel-type extension cord that was used to energize a clamp-on light fixture used at the Powermatic chain mortise, model 15.
