Text For Citation: 01 Item/Group: 002 Hazard:

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation and crushing hazards from a tenoning machine: Back Shop: On, or about, December 1, 2014, adequate work space was not provided around the Millbury Machine 5/8 inch tenoner. The operator was provided with approximately one foot of working space between the machine and the frame saw. Also, a makeshift bolt was used as a stop for the work table. The table fell off the machine when the table was retracted to the starting position.
