29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized, other safety related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a/ High frequency testing department (lower level of 4328 Bridgeton Industrial Park Drive, Bridgeton, MO 63044): The employer failed to provide employees with adequate safety related work practices while working with live leads during the high frequency testing of bias transformers. The voltage amounted to be 1,200 volts. One employee performing this task was electrocuted on or about November 11, 2014.
