Text For Citation: 01 Item/Group: 002 Hazard: LOCKOUT
Section 5(a)(1)of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Employees were exposed to Class 1 Division 1 environment due to the accessibility of a hydraulic valve in the operating room of the building This valve allows the activation of the lift mechanism while personnel are in this location. This places the employees in a hazardous position in relation to the explosive environment. a) Building 2299 #3 Glaze Barrel Among other methods one feasible and acceptable method to correct this hazard, if the operation is rebuilt, is to replace the past type of hydraulic valve in use with a lockout type of valve. This could be used by maintenance personnel for authorized repairs. The Key to this device should be regulated so access is controlled. Reference made to industry practice. Violation occurred 3 June 89.
