29 CFR 1910.269(o)(5)(iv): A test observer, capable of implementing the immediate deenergizing of test circuits for safety purposes, was not present, and means for immediate deenergizing of the test circuit were not in effect when employees were present in the test area during testing: a. On or about October 15, 2014, at the Reddick, Florida Substation, in the Doble test area, during the excitation testing, a test observer had not present, and the Doble test extension safety interrupt buttons for immediate deenergizing of the Doble test set were defeated by taping one button and converting one to a footswitch, exposing employees to a 10,000-Volt, 0.3-Ampere electrocution hazard.
