29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: (a) Mulching Area: The Pyro-Chem ABC portable fire extinguishers had not been subjected to a monthly visual inspection, as determined on March 5, 2014. (b) Shop Area: The portable fire extinguisher located next to the entrance/exit door had not been subjected to a monthly visual inspection, as determined on March 5, 2014. (c) Sawmill: The portable fire extinguisher located between the chop saw and the edger was not subjected to a monthly visual inspection, as determined on March 5, 2014. (d) Sawmill: The portable fire extinguisher located near the head saw was not subjected to a monthly visual inspection, as determined on March 5, 2014. ABATEMTNENT CERTIFICATION IS REQUIRED FOR THIS ITEM Wayne Mulch & Lumber, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, Portable Fire Extinguishers, 1910.157(e)(2): which was contained in OSHA inspection number 315223008, Citation number 1, Item number 3, group (b) and was affirmed as a final order on May 26, 2011, with respect to a workplace located at Wayne Lumber & Mulch, Inc. in Wayne, West Virginia.
