29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required. Project Light Inc. failed to provide documentation demonstrating the abatement methods and completion of each of the cited items described below: Citation 1 Item 1 - The warehouse floor along the north side of the building where the roof as leaking was not maintained in a clean and dry condition. Citation 1 Item 2 - Material damaged by water damage near the loading dock were not stored so that the material was stable and secure against sliding and collapse. Citation 1 Item 3 - Forklift operator training and certification. Citation 1 Item 4/a - Develop and implement a written hazard communication program. Citation 1 Item 4/b - Ensure secondary containers containing chemicals are labeled with the identity of the chemicals and hazards. Citation 1 Item 4/c - Maintain safety data sheets and ensure that they are readily accessible to employees. Citation 1 Item 4/d - Train employees on the hazardous chemicals in the workplace. Citation 2 Item 1 - Visually inspect fire extinguishers at least monthly. Citation 2 Item 2 - Train employees on the emergency action plan in case of fire. Citation 2 Item 3/a - The Dixie model HHMPS-10 Work Platform forklift attachment did not have written approval by the forklift manufacturer. Citation 2 Item 3/b - The Yale sit-down forklift was not equipped with weight capacity identification of the truck and the work platform attachment. Citation 2 Item 4 - Relocatable power taps and electrical junction boxes located at the assembly station were not installed in accordance to the manufacturer's listing and labeling.
