29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On June 23, 2015, employees were using an extension cord without a grounding pin, thereby exposing employees to an electrical shock. The employer did not ensure that the path to ground from circuits, equipment, or enclosures were permanent and continuous. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
