29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Rubber Department: A Safety Seal 4" double sided bench grinder did not have tongue guards on either side and the distance between the top of the peripheral member and the wheel measured 1/2 inch on each side, on or about 8/31/17. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. Armadillo Automation Inc. dba Onyx Valve was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 1097518, citation number 1, item number 4 and was affirmed as a final order on 9/15/16 with respect to a workplace located at 835 Industrial Highway, Suite 4, Cinnaminson, NJ 08077.
