29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the North Edge Grinder located at the Feilong Line, prior to conducting activities such as, but not limited to, unjamming the grinder. The North Edge Grinder was not isolated from the electrical energy sources prior to engaging in this maintenance activity. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. B) Autoneum North America, Inc., located at 645 Lallendorf Road, Oregon, OH 43616: On or about December 23, 2016, the employer did not ensure that employees implement energy control procedures on the equipment such as, but not limited to, the oven conveyor located at the Feilong Line, prior to conducting activities such as, but not limited to, unjamming the conveyor. The oven conveyor was not isolated from the electrical energy source prior to engaging in this maintenance activity. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conduct servicing activities such as, but not limited to, unjamming the grinders. Autoneum North America, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1910.147(d)), which was contained in OSHA Inspection # 1112707, Citation number 2, Item number 1, and was affirmed as a final order on March 16, 2016, with respect to a workplace located at 1150 North Wynn Road, Oregon, OH 43616. Autoneum North America, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard (1910.147(d)(3)), which was contained in OSHA Inspection # 1063461, Citation number 1, Item number 2, and was affirmed as a final order on August 18, 2015, with respect to a workplace located at 645 Lallendorf Road, Oregon, OH 43616.
