29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident: The employer is failing to report a work-related fatality within 8-hours. Most recently, on or about 04/05/2016, at the end of County Road 4410 in Boss, MO 65440 (Logging Site), an employee was fatality injured in a work-related incident. Notification of the fatality was conducted approximately 6 days after the incident on 4/11/2016.
