29 CFR 1926.503(a)(2)(viii): The employer did not assure that each employee had been trained by a competent person qualified in the standards contained in subpart M: On or about October 24, 2014 and prior to that time located at various points on the water tower employees were working without compliant fall protection systems being utilized performing work from powered single-point and two-point adjustable suspension scaffolds exposed to fall hazards in excess of 60' who were not trained on the applicable regulations or procedures to be followed to minimize those fall hazards.
