29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). The employer failed to develop adequate procedures for the elimination, isolation, restraint and relief of pneumatic energy sources to the outer metal door of the quench tank chamber of the AFE-6 Casemaster furnance during the insertion of an extended wrench into the quench tank chamber of the furnance to adjust the quench tank elevator.
