29 CFR 1910.269(l)(9): Fuse handling. When an employee must install or remove fuses with one or both terminals energized at more than 300 volts, or with exposed parts energized at more than 50 volts, the employer shall ensure that the employee uses tools or gloves rated for the voltage. A) Amtrak - Hellgate Substation 46: An employee changing fuses by hand on a high voltage (12.5kv or 25kv) lines to the substation transformer did not use the proper rated tools or insulated work gloves and used leather gloves. The employee was electrocuted; the employer did not ensure employees used tool or gloves rated for the voltage. On or about, December 5th, 2020. NOTE: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
