29 CFR 1910.119(f)(1)(ii)(B): The employer did not develop and implement written operating procedures that provide clear instructions for steps required to correct or avoid deviation from the operating limits in the covered process: Employees engaged in operating, inspecting, testing, and maintaining activities at the covered process equipment in the roof top, the CP-2 and CP-4 engine rooms were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility in that the steps required to correct or avoid the deviations listed within the operating procedures for Ammonia process equipment were inadequately documented, missing, unclear or incomplete.
