29 CFR 1910.146(j)(6): The employer did not ensure that each entry supervisor determined, whenever responsibility for a permit space entry operation was transferred and at intervals dictated by the hazards and operations performed within the space, that entry operations remained consistent with terms of the entry permit and that acceptable entry conditions were maintained: Employees entered tank 242 under a confined space permit to perform maintenance and cleaning; the maintenance work was not noted on the permit and the permit did not note that employees would be working above the floating roof where they could not be seen by the attendant.
