29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: a- Roof Level/ Material Storage: Employees were exposed to fall hazards while engaged in commercial roofing activities in which the safety monitor was engaged in roofing activities and not acting in the capacity of a monitor exposing employees to (6) unprotected sky lights and fall hazards of approximately 22ft to the concrete below. Condition noted on or about 10/12/2018 b- Roof Level/ Material Storage:Employees were exposed to fall hazards while engaged in commercial roofing activities in which the safety monitor was engaged in roofing activities and not acting in the capacity of a monitor exposing employees to (6) unprotected sky lights and fall hazards of approximately 22ft to the concrete below. Condition noted on or about 10/13/2018 c-Roof Level/ Material Storage: Employees were exposed to fall hazards while engaged in commercial roofing activities in which the safety monitor was engaged in roofing activities and not acting in the capacity of a monitor exposing employees to (6) unprotected sky lights and fall hazards of approximately 22ft to the concrete below.. On 10/14/2018, an employee fell through an unprotected sky light and suffered fatal injuries as a result of the fall.
