On or about August 9th 2016 in the Multi-Purpose Unit at Arkema, Inc. located in Crosby, TX inspections and tests were not performed on process equipment, that include but are not limited to the following: a) SO2 detector(s) located at SO2 tank b) LEL detector(s) in Building 30 by T-24, c) LT-4183, d) LT-4061 e) TI/TT-4404, f) TI/TT-4403, g) TI/TT-4136, h) TI-4086A, i) TI-4086B, and j) Pipeline 1 ½ -CP-1399-316L from 17-T-19 (Sulfur dioxide tank) to EH-4 (SO2 Vaporizer) k) Pipeline 1-P301009-P from Pump P-63A(P&ID CR-30-X-0012 Rev. D) to Heat Exchanger 31-EH-14 (P&ID CR-30-X-0100 Rev. B)
