29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: On or about May 27, 2015, the employer did not ensure prior to performing die changes, material changes, and the changing of punches inside the point of operation of a Niagara 50 ton, part revolution mechanical power press, Serial No. 30223, known as the Mini Line #4, authorized employees verified isolation and de-energiztion of the press had been accomplished.
