29 CFR 1910.1200(g)(2)(iii): As required by 29 CFR 1910.1200; Appendix D of the standard Table D.1. Heading 3. Subheading For Mixtures in addition to the information required for substances (a) through (d). The chemical manufacturer or importer preparing the safety data sheet did not ensure that all the chemical names or concentration to make up the with known health hazards were listed and identified within Section 3 Composition Information on Ingredients of the safety data sheet. On or about November 11, 2020, an explosion occurred during the manufacturing of PureVista a water-activated product used to generate chlorine dioxide and the employer did not list or identify all names and/or concentrations for each ingredient used to make up the mixture inside the product's safety data sheet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
