On or about November 11, 2020, an explosion occurred during the manufacturing of PureVista a water-activated product used to generate chlorine dioxide and the employer did not list or identify all names and/or concentrations for each ingredient used to make up the mixture inside the product's safety data sheet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
