29 CFR 1926.417(b): Equipment or circuits that were de-energized did not have tags attached at all points where such equipment or circuits could be energized: On or about February 21, 2019, and at times prior thereto, two employees were involved in the replacement of an irrigation pump submerged in approximately 55 feet of lake water without tagging out the circuit that provided energy to the pump, exposing the employees to an electrical shock hazard.
