29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) South wall of building, third floor, on or about 5-19-14: Employees were working in the vicinity of an 8 foot wide door opening and were not protected from falls, exposing the employees to a fall hazard 16 feet 9 inches. b) South wall of building, third floor, on or about 5-19-14: Employees were working in the vicinity of a 36 inch wide window opening and were not protected from falls, exposing the employees to a fall hazard 16 feet 9 inches. c) South wall of building, second floor, on or about 5-19-14: Employees were working in the vicinity of a 6 foot wide door opening and were not protected from falls, exposing the employees to a fall hazard 8 feet 9 inches. d) South wall of building, second floor, on or about 5-19-14: Employees were working in the vicinity of a 3 foot wide window opening with a top rail and no mid rail, exposing the employees to a fall hazard 8 feet 9 inches. e) North wall of building, second floor, on or about 5-19-14: Employees were working in the vicinity of two 6 foot wide door openings, both with top rails and no mid rail, exposing the employees to a fall hazard 8 feet 9 inches to the water below.
