29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: The employer is failing to protect employees from electrical shock hazards while using extension cords. This was most recently observed on May 13, 2020 at 530 Carl St, Butte, Nebraska where an extension cord was being used with a metal box designed for fixed installations. The metal box had knockout locations which increase the potential for damage to the box, exposing the live parts within.
