29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Lumber/Mill Yard (Facility): On or about August 01, 2017, and at times prior thereto the employer exposed employees to potential hazardous energies such as electrical, pneumatic and hydraulic in that employees were allowed to work on machines such as but not limited to the Gang Saw, Head Saw, Debarker, Cut up System, and Planer Mill Trim Saw without a written machine specific lockout/tagout (LOTO) procedures to ensure that energy sources were controlled while the employees were performing maintenance or service on the machine(s).
