29 CFR 1910.147(d)(6):Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished. a) Cooper Tire & Rubber Co., North Air Dryer - On or about November 4, 2020, employees installing a valve on the intake header of the air dryer did not verify if the air dryer was depressurized before attempting to remove a coupler and flange from the intake header, exposing employees to a struck-by hazard.
