29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck was dismounted and within 25 ft. of the truck still in his view, the load engaging means was not fully lowered. The employer is failing to prevent employee exposure to struck-by / crushing hazards. At 6190 Mid Rivers Mall Drive in St. Peters, MO 63304 - On or about 7/23/19, the operator was exposed to the struck-by / crushing hazard when he dismounted and was within 25 ft. of the truck still in his view, with the load elevated approximately 6 to 7 feet above the concrete floor. The "Crown" (Model No. RR5725-45; Serial No. 1A422917) powered industrial truck (PIT) was loaded with about 26 sheets of 5/8-inch drywall, measuring 48 inches in width by 144 inches in length, and had a combined weight of approximately 2,745.6 pounds.
