29 CFR 1910.119: The employers' PHA failed to address 29H-4 facility siting issues for the following: a) Delaware City Refinery, 4550 Wrangle Hill Road, Delaware City, DE, 19706 - Desulfurization Unit, Train 4, Heater 29-H-4:  On or about March 11 2020, the employer failed to address safe access to the fuel gas supply shutoff valves (FCV-169 and FCV-172) located on the north (oil-side) and south side (hydrogen side) of fired heater (29H-4), respectively. FCV-169 and FCV-172 are located approximately 10 feet from 29H-4 and are opened/closed manually by an operator to provide fuel gas to 29H-4. This violation contributed to a significant fire inflicting serious burn injuries on two operators and continues to create a fire/explosion hazard. b) Delaware City Refinery, 4550 Wrangle Hill Road, Delaware City, DE, 19706 - Desulfurization Unit, Train 4, Heater 29-H-4:   On or about March 11, 2020, the employer failed to address safe access to firewater monitor located on the east side of 29H-4 in the event of tube rupture. This violation contributed to a significant fire inflicting serious burn injuries on two operators and continues to create a fire/explosion hazard.
