29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) LACOM MPBL CV 1800 laminator; On or about July 24, 2019 and times prior, the employer exposed employees to the hazards associated with getting caught-in the feed rolls during threading operations of the lower rollers due to the 1800 laminator not being locked out to control the energy sources present such as electric and pneumatic.
