29 CFR 1910.303(g)(2)(ii): In locations where electric equipment is likely to be exposed to physical damage, enclosures or guards shall be so arranged and of such strength as to prevent such damage. (a) Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line, operator's single stop control box: on or about March 27, 2018, process checkers/inspectors were exposed to contact with live wires likely to result in serious injury when walking-by and performing general housekeeping tasks around taped, terminated ends of a flexible cable. The termination point was not guarded against accidental damage and contact by use of approved cabinets or enclosures. (b) Plant no. 9 (Streator, Illinois), selecting area, H2 FP3 inspection line's infeed conveyor: on or about March 27, 2018, process checkers/inspectors were exposed to contact with exposed live wires likely to result in serious injury when walking-by and performing general housekeeping tasks around a plugged-in flexible cable that had its outer jacket insulation damaged by the plug connection. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
