29 CFR 1926.1203(e)(2)(v)(A): Continuous forced air ventilation was not used when an employee entered a space and until a hazardous atmosphere had been eliminated: (a) At the construction site on Belfast Keller and Interstate I-95, on or about July 8, 2019, the employer did not ensure that forced air ventilation was used inside an underground water main vault before employees entered the space, exposing employees to an oxygen deficient atmosphere.
