29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) On or about September 14th, 2017 and at times prior employees were exposed to cave-in hazards while installing a water line in an unprotected trench approximately nine and a half feet deep in the vicinity of 4099 South ND Highway 3 in Dawson, North Dakota.
