29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: Jobsite located at 158 Cronin Avenue, Forestville, Connecticut: On, or about, May 4, 2015, the company had not provided effective training to employees whose job duties required entry into permit required confined spaces. Although Supervisors and field employees possessed a basic awareness of confined spaces, they had limited working knowledge of the requirements of the OSHA regulation and of the company's internal procedure TD955 entitled Confined Space Requirements.
