29 CFR 1910.146(d)(3)(iv): Develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to, the following: (iv) Purging, inerting, flushing, or ventilating the permit space as necessary to eliminate or control atmospheric hazards: On or about June 7, 2021, Employees were exposed to atmospheric hazards while entering sewer manholes due to the fact the Employer failed to ensure manholes were adequately ventilated and failed to develop and implement practices and procedures related to when powered ventilation shall be used during entries into sewer manholes and other permit required confined spaces.
