29 CFR 1926.1203(e)(2)(iii): Before an employee entered the space, the internal atmosphere was not tested with a calibrated, direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants. (a) At the construction site on Belfast Keller and Interstate I-95, on or about July 8, 2019, the employer did not ensure that the internal atmosphere was tested inside an underground water main vault before an employee entered the space, exposing employees to an oxygen deficient atmosphere.
