29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces: Conveyor 81A: On, or about, July 10, 2018, the access cover to the screener section of the machine was provided with a sign that identified the space as a non-permit required space. Using both the guidelines in 29 CFR 1910.146 and the employer's Confined Space Determination Worksheet, the screener section is classified as a Permit Required Confined Space due to the internal configuration of the space, a fall hazard in the space and the requirement that employees stand on rotating conveyor shafts that are required to be locked out.
