29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms and the entire platform was not attached to the fork and the fork-lift is not moved horizontally while the platform is occupied: On or about November 15, 2011 and at times prior thereto, employees were exposed to falling approximately 16 feet to the ground below while installing siding from scaffolding that was supported by a ladder jack on one end and a fork lift on the other.
