29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a)Quality Roofing by Petitt, Inc. at 451 East Boardwalk Drive, Blding 11, Fort Collins, CO 80525: On and before February 28th, 2014, the employer did not ensure that employees shingling a three story apartment building with a 4.5 in 12 pitch roof used fall protection. The condition exposed employees to fall hazards of up to 25 feet from ground to eave.
