29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: (a) Mulch Yard - Colorbiotics Sahara X3 Coloring Machine - On or about 03/3/2021, employees were walking and working on top of the Colorbiotics Sahara X3 Coloring Machine while performing daily checks, maintenance, and clearing jams without fall protection, exposing employees to falls of approximately 8.5 feet to the ground off of the sides of the coloring machine and the end of the coloring machine opposite the hopper.
