29 CFR 1928.57(d)(1)(ii): When guarded by railings, any hazardous component within 15 horizontal inches of the rail was not completely enclosed. Railing height was not 42 inches off the floor, platform, or other working surface, with a midrail between the top-rail and the working surface. . In the washplant sorting area : on or about August 19, 2020, employer did not ensure to install a middle rail between the top rail and the working surface. The top guard rail was not at least 42 inches high from the working surface. This condition exposed employees to struck-by hazards.
