29 CFR 1926.602(b)(3): Safety requirements, ratios, or limitations applicable to machines or attachment usage covered in Power Crane and Shovel Associations Standards No. 1 and No. 2 of 1968, and No. 3 of 1969 were not complied with: On or about 1/16/2020, at Custom Built Marine Construction Inc. worksite located at 915 Cordova Road, Fort Lauderdale, FL 33326. A front end loader (i.e. Komatsu WA270-7, Serial number 80562) equipped with a boom attachment was loaded over its capacity while pulling piles out of water, exposing employees to struck by and drowning hazards.
