29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) ADA Dash Main Line - A900; On or about June 17, 2016 and at times prior thereto, Employee #30 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock. b) ADA Dash Subline; On or about June 17, 2016 and at times prior thereto, Employee #1 was exposed to caught-in, struck-by, and crush hazards in that the employee was required to enter the robotic cell with energized robots and other jig and fixtures to replace a welding tip on a robot. The employee was unable to lock out the robotic cell because the employer had not provided or ensured the employee had a lock.
