29 CFR 1926.454(a): The employer does not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold in use and to understand the procedures to control or minimize those hazards. This violation was most recently observed on 7/30/2015 at a worksite at 51 Pleasant Street, Woburn, MA, where the employer allowed employees to work on scaffolds without adequate training by a person qualified in the subject matter.
