29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 5/5/15 and 5/7/15 in the bending machines area at 6144 Highway 341 South in Fort Valley, GA: Employer failed to provide clear and specific written energy control procedures for authorized employees that were required to troubleshoot, adjust, set up, and perform service and/or maintenance on the bending machines without locking and or tagging out all energy sources. One employee was injured on 5/5/15 while trying to correct the malfunctioning bending machine.
